We are seeking a skilled and compassionate Personal Care Specialist to provide quality care to medically fragile patients in the comfort of their own homes.

Key Responsibilities

  • Provide high-quality, personalized care to patients with complex medical needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare providers to ensure seamless care coordination
  • Maintain accurate and detailed records of patient care and progress

Requirements

  • Certified as a Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN)
  • Experience working with pediatric patients and/or those with tracheostomies and ventilators
  • Ability to lift up to 50 pounds and stand for extended periods
  • Valid driver's license and reliable transportation
