Job Overview

We are seeking a highly skilled Medical Care Coordinator to join our team at Easton Home Health. This is a full-time opportunity for a Licensed Practical Nurse (LPN) to provide quality patient care and technical skilled care in the comfort of the patient's own home.

About the Role

The LPN will be responsible for providing services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other members of the healthcare team. The ideal candidate will have excellent communication skills, be able to work independently, and have a strong commitment to delivering exceptional patient care.

Responsibilities
  • Provide quality patient care and technical skilled care in the patient's home.
  • Instruct patients' families in the care of the patient and maintenance of a healthy environment.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
Requirements

To be considered for this role, you must have:

  • A current LPN licensure in the state of practice.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification.

Salary and Location

This is a full-time opportunity with an estimated salary range of $65,000 - $80,000 per year, depending on experience. The position is based in Easton, PA, and may require travel to various locations throughout the region.
