We are seeking a skilled Nurse Practitioner to join our team in Danville, VA. As a PRN Nurse Practitioner, you will work autonomously to perform annual comprehensive health assessments for Medicare age patients in the home. This is a flexible opportunity with a mileage reimbursement plan and the ability to create your own schedule.
Responsibilities:
- Perform annual comprehensive health assessments in the home
- Work with Medicare age patients to identify and address health risks
- Develop tailored lifestyle improvement plans to support patient health
Requirements:
- Board Certified Family Nurse Practitioner, Acute Care Nurse Practitioner, or Adult-Geriatric Nurse Practitioner
- Experience in urgent care or ambulatory care preferred
- New grad Nurse Practitioners welcome to apply
