At Abode Care Partners, we're seeking a compassionate Medical Care Specialist to provide personalized medical care to seniors and patients with complex needs. As an integral member of our team, you will collaborate with healthcare professionals and caregivers to evaluate and treat various medical conditions.

Responsibilities:
  • Deliver Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs to ensure the best possible outcomes.
  • Treat Chronic and Urgent Conditions: Examine and treat minor injuries, accidents, and other illnesses while maintaining accurate patient records.
  • Collaborate with Healthcare Professionals: Work alongside caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
  • Refer Patients as Needed: Provide referrals to other healthcare practitioners when necessary to ensure seamless care coordination.
Qualifications:
  • Nurse Practitioner (NP) License: Active state NP license required for this role.
  • Master's Degree in Nursing: A Master's degree in Nursing (MSN) is essential for this position.
  • Experience and Skills: Five or more years of nursing experience; at least two years of NP experience preferred. Strong interpersonal communication skills, excellent time management skills, and proficiency in medical instruments and equipment are also required.
