Ambulatory Clinical Pharmacist: Specialty Pharmacy, Digestive Health Expert

As a Clinical Pharmacist in Ambulatory Care, you will play a vital role in ensuring the safe and effective use of medications for patients in the UVA Medical Center. Your expertise will be instrumental in assessing medication orders, selecting optimal drug products, designing effective dosing regimens, and monitoring drug therapy. You will work closely with medical staff to identify alternatives and ensure the best possible care for patients.
Key Responsibilities:
- Provide medication order verification and assessment for optimal drug product selection, dosage regimen design, and effective administration.
- Monitor inpatient and outpatient drug therapies, collaborating with medical staff to find alternatives when needed.
- Deliver clinical pharmaceutical care to patients, enhancing the quality of care delivery.
- Demonstrate proficiency in Pharmacy distributive services.
- Support the educational and professional development of the Pharmacy department.
- Engage in professional self-development.
Requirements:
- Bachelor's in Pharmacy or Pharm. D Degree required.
- PGY-1 residency required or equivalent hospital experience. PGY-2 residency is preferred for specialty areas.
- Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required.
