Job Summary

We are seeking a skilled Home Health Nurse Specialist to join our team. As a key member of our care team, you will provide high-quality patient care in the comfort of their own homes. Responsibilities
  • Implement physicians' prescribed plans of care with precision and attention to detail.
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals.
  • Administer prescribed medication, treatments, and therapies as ordered.
  • Ensure timely and compliant documentation of care provided in our system of record.
  • Health promotion, teaching, and training of family members are essential aspects of this role.
  • Frequent communication with physicians, clients, family members, staff, and case managers is necessary to address client problems, concerns, and changes in client condition.
Qualifications
  • Maintain an active, unencumbered license (RN/LPN/LVN) in the state where you practice.
  • Current CPR certification from American Red Cross, American Heart Association, or BLS is required.
  • TB skin test within the last 12 months is mandatory.
  • Six months prior hands-on nursing experience is preferred but not required.
  • Reliable transportation is essential for this role.
