We are seeking a highly skilled Rehabilitation Case Manager to join our team at Christian Living Communities. This role plays a vital part in ensuring the successful transition of residents from our skilled nursing community into their homes, while promoting a seamless continuum of care.

About the Role

The ideal candidate will have a strong background in nursing and excellent leadership skills, with a proven track record of experience in senior services. As a Rehabilitation Case Manager, you will be responsible for coordinating care transitions, developing coaching relationships with residents and stakeholders, and empowering residents to actively participate in their plan of care.

Responsibilities

">
  • Care Coordination: Coordinates the work of team members to meet resident goals and expectations.
  • Post-Admission Screening: Performs post-admission screening to determine the resident's level of current knowledge of disease processes.
  • Coaching Relationships: Develops a coaching relationship with the resident and relevant stakeholders, empowering them to actively participate in the plan of care.
  • Education and Support: Follows up with required communication, support, and education with residents/family and team members to reduce the risk of readmission post-discharge.
  • Care Conferences: Coordinates and evaluates care conferences, care plan updates, and Med A meetings, including discharge plan and rehab and transition goals.
  • Discharge Planning and Case Management: Oversees discharge planning and case management.
  • Resident Education: Implements all resident education materials and disease management teaching.
  • Communication: Ensures effective communication both internally and externally to foster continuity of care.
  • Quality Improvement: Evaluates resident readmissions and makes recommendations for quality improvement practices.
  • Palliative/Hospice Care: Identifies residents in need of advanced care planning and initiates palliative/hospice care.
  • Clinical Oversight: May assist with clinical oversight as required.

Qualifications and Experience

  • Nursing Background: Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing required.
  • Leadership Skills: Minimum of 3 years in a supervisory capacity required.
  • Knowledge of Healthcare Regulations: Must be knowledgeable of state and federal laws and regulations for skilled nursing facility operation in the state of Colorado.
  • Administrative Experience: Administrative experience preferred.
  • Computer Skills: Must have computer word processing and database experience.
  • Customer Service: Must possess excellent customer service skills, interacting with medical professionals, residents, family members, and care partners.
  • Language Skills: Must be able to read, write, and speak the English language.

Benefits and Working Conditions

  • Salary Range: Estimated salary range: $75,400 - $99,200 per year, based on location and experience.
  • Benefits: Health coverage, health savings accounts, retirement (with match), dental, vision, disability, and life insurance, paid time off plan.
  • Working Conditions: Light physical effort required, sitting, standing, bending, and moving intermittently during working hours, frequent interruptions, and interaction with donors, families, volunteers, residents, and staff members.
