Job Summary

We are seeking a skilled Care Coordinator to join our team in Delaware. As a member of our community care team, you will work closely with clients and healthcare professionals to provide comprehensive care coordination services.

About the Role
  • You will assess client needs and develop personalized care plans to ensure optimal outcomes.
  • Collaborate with healthcare providers, family members, and caregivers to coordinate care across settings.
  • Educate clients and caregivers about available resources and support services.
  • Travel to clients' homes, nursing facilities, and other community-based settings to conduct face-to-face assessments and provide ongoing support.
Requirements
  • A current Registered Nurse (RN) license in the state of Delaware.
  • At least 2 years of experience in case management and discharge planning, including experience working in a facility setting.
  • Strong communication and interpersonal skills to effectively collaborate with clients, families, and healthcare teams.
  • Proficiency in using electronic documentation systems and database software.
  • Able to travel within assigned territory and work flexible hours as needed.
