LPN (Licensed Practical Nurse)

The Ready 4 Work team is seeking a skilled LPN (Licensed Practical Nurse) to provide high-quality nursing care to our residents. As a key member of our healthcare team, you will be responsible for maintaining accurate nursing documentation, administering medications, and providing wound care, tracheostomy care, and diabetic counseling. You will also assess residents' health and emotional needs, communicate effectively with other staff members, and maintain confidentiality at all times.
Key Responsibilities
- Maintain accurate and up-to-date nursing documentation and medication records
- Administer medications as ordered by physicians and record in electronic MAR
- Provide wound care, tracheostomy care, and diabetic counseling as needed
- Assess residents' health and emotional needs and develop individualized care plans
- Communicate effectively with other staff members, including nursing and program staff
- Maintain confidentiality and adhere to HIPAA guidelines
Requirements
- Current LPN license
- Ability to work effectively in a fast-paced environment
- Excellent communication and interpersonal skills
- Ability to maintain confidentiality and adhere to HIPAA guidelines
