Salary:$60,000 - $80,000 per year, negotiable based on experience and education

Job Description:

This position is responsible for helping individuals understand, navigate, and grow through the Comprehensive Community Services (CCS) program. As a Compassionate Care Specialist, you will meet and work with individuals, service providers, and other team members to assist in their progression through the CCS program.

Responsibilities:

  • Establish individual's eligibility by using a Mental Health/AODA Functional Screen
  • Assist the individual in identifying services from the CCS service array appropriate to address recovery needs
  • Broker services as needed with other providers in the CCS network
  • Work with the individual to identify and build a recovery support team of professionals and natural supports
  • Develop and implement an individualized recovery treatment plan
  • Facilitate recovery meetings with the individual and their team
  • Work with the individual to prioritize and obtain goals set forth in the individual recovery plan
  • Follow-up with service providers to ensure quality care and goal progression
  • Document all contact with individuals and team members

Required Skills and Qualifications:

To be successful in this role, you will need:

  • Bachelor's degree in Social Work or a related field
  • Two years of experience working in the Mental Health field
  • Experience working with people with substance use needs preferred
  • Ability to maintain and promote positive working relationships within WCS and with external partners
  • Excellent assessment and interviewing skills
  • Strong understanding of mental health and substance use needs
  • Ability to work effectively as a member of a team
  • Effective use of time and resources to maintain quality CCS services

This role requires:
