About the Role:

We are seeking a highly skilled Nurse Practitioner to join our team at Abode Care Partners.

The estimated annual salary for this position is around $110,000-$130,000 based on industry standards and location.

Job Description:

  • Provide evidence-based medical care to seniors and patients with complex needs in various settings.
  • Examine, treat, and manage chronic and urgent health issues while collaborating with healthcare teams to maintain comprehensive care plans.

Responsibilities:

  • Perform medical care planning, direction, and evaluation of patient medical needs.
  • Examine and treat episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
  • Provide referrals to other healthcare practitioners when necessary.
  • Maintain up-to-date patient medical records.

Qualifications:

  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills – verbal and written.
  • Excellent time management skills with accountability for practitioner's daily workflow.
  • Proficiency in medical instruments and equipment.
  • Knowledge base of HIPAA and OSHA regulations.
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
  • Licensed driver with valid automobile insurance.
  • Ability to engage in moderate physical activity.
