Care Coordination Manager

**Job Summary**

We are seeking a highly skilled Care Coordination Manager to join our team at FCS, Inc. As a key member of our organization, you will be responsible for coordinating care services for individuals and families in need.

**Responsibilities**

* Coordinate care services with healthcare providers, social services, and other stakeholders to ensure comprehensive support for clients
* Develop and implement care plans that meet the unique needs of each client
* Collaborate with multidisciplinary teams to ensure seamless care coordination
* Provide education and support to clients and their families on mental health issues and available resources
* Maintain accurate records and reports to track client progress and program outcomes

**Requirements**

* Master's degree in a recognized mental health discipline preferred
* Bachelor's degree in a recognized mental health discipline with work experience in the human services field required
* LCSW/Licensed Professional Counselor, etc.
* At least 2 years of experience in the mental health/human services field
* Must successfully pass the KBI, SRS Central Registry, and all other background investigations
* Experience working with children and families preferred
* Full-time, Monday through Friday schedule
* Bilingual English and Spanish a plus
