Key Responsibilities
Review health plans of each resident to ensure accurate and effective care.
Develop and manage the self-administration of medication program for residents, ensuring proper documentation and adherence to regulations.
Act as a liaison between physicians and program staff to ensure seamless communication and care coordination.
Provide training to residents and staff on hygiene, medication side effects, first aid, and nutrition.
Participate in treatment team meetings to discuss resident care and progress.
Requirements
Valid Pennsylvania license as a Licensed Practical Nurse (LPN) with experience working with mental health populations, psychotropic medications, and the chemically dependent.
Strong communication and interpersonal skills, with the ability to work effectively with residents, staff, and physicians.
Ability to maintain confidentiality and handle sensitive information with discretion.
Flexibility to work varying shifts, including evenings and weekends.
