Care Coordinator for Individuals with Intellectual and Developmental Disabilities

About this roleWe are seeking a highly skilled Licensed Vocational Nurse (LVN) to join our team as a Care Coordinator for individuals with intellectual and developmental disabilities. As an LVN in this role, you will work closely with a dedicated team to deliver high-quality nursing services, training, and support to individuals with complex needs.
Key Responsibilities:- Monitor health conditions, ensure quality services, and provide documentation
- Administer medications, oversee staff members, and provide prescribed medical treatment
- Conduct assessments, implement healthcare services, and respond to changes in condition
- Provide training to staff on universal precautions, infection control, and medication administration
- Serve as liaison and advocate with other medical and professional service providers
- Associate's Degree in Nursing or related field
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure
- Ability to effectively communicate with staff, individuals served, and families
