Inpatient Clinical Pharmacist IV, Critical Care and Cardiology Specialist

Job Summary:
We are seeking a highly skilled Inpatient Clinical Pharmacist IV to join our team at Sutter Health. As a critical care and cardiology specialist, you will be responsible for delivering pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost-efficient medication therapy.
Key Responsibilities:
- Deliver pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost-efficient medication therapy.
- Exercise sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner.
- Precept Pharmacy Residents in an accredited Residency Program.
- Gain confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication.
- Adhere to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care.
Requirements:
- Doctorate: Graduation from an accredited school of Pharmacy.
- CERTIFICATION & LICENSURE: PHARMD-Licensed Pharmacist or PHARMR-Registered Pharmacist. PGY1 and PGY2 in Critical Care or Cardiology Preferred. Board Certification BCPS or BCCCP Preferred.
- TYPICAL EXPERIENCE: 3 years recent relevant experience.
- SKILLS AND KNOWLEDGE: Ability to influence physician prescribing at the service-line level. Demonstrated skills in managing projects and initiatives which have impact at the facility level. Must meet qualifications to precept Pharmacy Residents in an ASHP accredited Residency Program. Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations and laws and regulations. Knowledge and understanding of different Pharmacy practice settings, including narcotic delivery and procedures.
Working Conditions:
This position may regularly work, store, prepare, receive, unpack, transport, dispose of, or administer drug(s) identified as hazardous, or potentially hazardous, by the National Institute for Occupational Safety and Health (NIOSH) for purposes of USP 800.

Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs.
