About the Role:

We are seeking a skilled Healthcare Outreach Specialist to join our team at Grane Home Health & Hospice. As a key member of our community outreach team, you will be responsible for building and maintaining professional relationships with referral sources, including hospitals, physician and surgeon practices, nursing homes, and other healthcare organizations.

Key Responsibilities:

  • Establish and maintain professional relationships with referral sources to promote our home health and hospice services.
  • Ensure compliance with all state, federal, and local regulatory requirements.
  • Understand the benefits of home care to both professional referral sources and consumers.
  • Educate the community about home care services and Abode Home Health.
  • Interact daily with professionals in the healthcare industry, including physicians, discharge planners, medical social workers, case managers, and skilled nurses.
  • Conduct outside sales calls to existing and potential referral sources within an established territory.
  • Promote Abode Home Health's programs and services to medical professionals and civic and community-related organizations.
  • Attend trade shows and vendor fairs to generate business and establish professional relationships.

Requirements:

  • Bachelor's degree in Business, Healthcare, or other related discipline preferred.
  • Current and valid state driver's license.
  • Proof of auto insurance.
