Job Summary

We are seeking a skilled and compassionate Medical Care Coordinator to join our team at ConnextCare. As a key member of our healthcare team, you will play a vital role in ensuring the highest level of patient care and satisfaction.

Key Responsibilities
  • Escort patients into examination rooms and initiate chart documentation, adhering to all local, state, and federal requirements.
  • Maintain a timely flow of patients according to the day's schedule, ensuring efficient use of resources.
  • Provide patient education materials and facilitate patient questions and answers, promoting informed decision-making.
  • Execute medical regimens prescribed by licensed providers, including tests, treatments, and referrals, as well as prescription preparation and computer scheduling.
  • Complete the appropriate area of the electronic health record based on patient scheduled visit, including adequate charges for services provided.
  • Perform tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse or licensed physician.
  • Participate in quality improvement, in-service workshops, continuing education, and peer review as requested by management and supervision.
  • Follow up on provider requested orders related to lab results and specialist referrals, ensuring seamless communication.
  • Comply with all OSHA regulations, CLIA, NYSDOH, Quality & NOCHSI policies and procedures, maintaining a safe and healthy work environment.
  • Engage in Patient Centered Medical Home initiatives, including participating as part of the Clinical Care Team.
Requirements
  • Graduate of a certified nursing program, with a New York State license to practice practical nursing.
  • Ability to work well and with compassion in stressful situations, maintaining a professional and courteous demeanor with both patients and co-workers.
  • Keen awareness of the importance of confidentiality in all aspects of the position.

Salary: $/hr. Depending on Experience
