Nursing Care Transition Coordinator

We are seeking a seasoned Admissions Clinical Liaison to fill a full-time position at Marshall Rehab & Nursing.
About the RoleThis is an excellent opportunity for an experienced professional to join our team as a skilled nursing admissions clinical liaison. The successful candidate will be responsible for initiating admission assessments on new patients, conducting thorough medical evaluations, and ensuring all necessary forms and insurance obligations are completed.
The Admissions Clinical Liaison will work closely with other staff members to improve processes and ensure seamless patient care transitions. Key responsibilities include performing patient intake evaluations, clinically approving admissions, processing health insurance information, conducting background checks, and collaborating with the healthcare team to optimize patient outcomes.
The ideal candidate will possess experience in authorizations and insurance, familiarity with post-acute care settings, rehabilitation hospitals, and sub-acute rehabilitation facilities.

Salary InformationThe estimated annual salary for this position ranges from $70,000 to $90,000 based on relevant experience and qualifications.
Required Skills and Qualifications- Experience in admissions clinical liaison roles or similar positions in post-acute care settings
- Familiarity with electronic health records (EHRs) and practice management systems
- Strong communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams
- Ability to work independently and manage competing priorities in a fast-paced environment
