Hospital Care Coordinator

The ability to work collaboratively with physicians, office staff, and ancillary departments is also required.

Job Description:
- Facilitate care along a continuum through effective resource coordination
- Assess patients for transition needs and identify those at risk for readmission
- Conduct complex psycho-social assessments and interventions to promote timely throughput and safe discharge
- Develop and implement transition plans to prevent avoidable readmissions
- Collaborate with physicians, office staff, and ancillary departments to ensure seamless care coordination
- Maintain accurate and timely documentation in the Case Management system
- Participate in department quality improvement initiatives
Required Qualifications:
- Master's of Social Work degree
- At least 2 years of acute hospital experience
- Certified or registered to practice profession as required by law or regulation in state of practice or policy
