Job Summary

We are seeking a skilled Kidney Care Coordinator to join our team at Healthmap Solutions.

About the Role

This is a unique opportunity to make a meaningful impact in the lives of individuals with chronic kidney disease. As a Kidney Care Coordinator, you will play a critical role in assisting patients with care coordination and complex care management duties.

You will develop and maintain individualized care plans, working closely with healthcare providers and community resources to ensure seamless care transitions. Your exceptional communication skills and ability to build strong relationships will be key to driving positive patient outcomes.

Duties and Responsibilities
  • Assess patients in their homes or other settings to identify their needs and develop personalized care plans.
  • Evaluate home environments for safety and infection control, as well as identifying opportunities for community resource support.
  • Provide first-level support for patients to resolve issues and improve their overall experience.
  • Partner with interdisciplinary care teams to drive improvement in patient health outcomes.
  • Act as a liaison between community resources, programs, and relevant services.
Requirements

To succeed in this role, you will need:

  • A Master's degree in nursing, preferably with a focus on nephrology or related field.
  • An active, unrestricted RN license, with nurse practitioner or advanced practice nurse credentials (as required by state).
  • At least 3 years of experience in care management roles, preferably within nephrology practices, dialysis centers, or transplant centers.
  • Prior experience building and managing relationships with healthcare providers or patients.
Salary and Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience.

In addition to a comprehensive benefits package, including medical, dental, and vision coverage, as well as paid time off and retirement savings options.

Travel requirements: This position involves travel over 50% of the time, primarily within the Tampa Bay area.
