Job Summary

We are seeking an experienced Patient Care Coordinator RN to join our team at All Medical Personnel.

About the Job:

  • Provide comprehensive patient care coordination services, ensuring seamless transitions between healthcare settings.
  • Collaborate with physicians and other healthcare professionals to develop individualized care plans that meet patients' unique needs and preferences.
  • Utilize expertise in InterQual and Millman to facilitate informed decision-making regarding patient care.
  • Lead discharge planning efforts, ensuring timely and effective transfers to post-acute care settings.
  • Prioritize patient safety and quality of care, advocating for patients' rights and interests throughout the care continuum.
Requirements

To be successful in this role, you will possess:

  • A minimum of 5-6 years of experience in case management within an inpatient/acute care hospital setting.
  • Demonstrated expertise in InterQual and Millman, as well as discharge planning principles.
  • The ability to work effectively with diverse patient populations, addressing their unique needs and preferences.
  • Certification in Case Management or Utilization Review is highly desirable.
  • Experience with electronic medical records systems, such as MediTech, is a plus.

Salary Information

The estimated salary range for this position is between $85,000 and $110,000 per year, based on industry standards and location considerations.
