Job Summary

We are seeking a skilled Social Worker to join our team at Fairview Health Services. As a key member of our healthcare team, you will provide a wide range of social work services and interventions to assist individuals, families, and communities in restoring, enhancing, or protecting their capacity for productive social functioning and coping with their illness and healthcare status.

Key Responsibilities
  • Plan and implement treatment and transition care plans based on a psychosocial assessment integrated into an interdisciplinary care plan.
  • Provide a range of social work interventions, including case management, counseling, crisis intervention, and advocacy, to assist patients and families in reaching their goals.
  • Identify and recognize rapidly changing situations and quickly intervene to prevent or resolve emotional crises.
  • Arrange, mobilize, utilize, and coordinate a comprehensive range of community resources to meet the identified patient and family needs.
  • Take an active role in facilitating interdisciplinary communication regarding patients and families experiencing complex psychosocial issues.
  • Formally and informally educate patient/family and healthcare team members regarding the social work role, pertinent patient care issues, and population-specific information.
  • Serve as a preceptor or field instructor for social work undergraduate training as appropriate to their skills related to a range of psychosocial issues and interventions.
  • Serve as a patient advocate and liaison with physicians, families, and community to ensure continuity of care.
  • Document psychosocial assessment, progress notes, care plans, and discharge/transition plan in the electronic medical record.
  • Identify and participate in Quality Improvement, research, and program evaluation initiatives that focus on improving healthcare services.
  • Demonstrate knowledge and compliance with MHealth Fairview and Social Work practice, ethics, and standards.
  • Comply with legal and regulatory laws, including mandated reporting requirements.
Requirements
  • Bachelor's degree in social work from an accredited social work program.
  • Current License Social Worker (LSW) through Minnesota Board of Social Work.
  • One to three years of experience in a healthcare setting and demonstrated knowledge of community and economic resources.
  • MSW or LICSW licenses.
