FeldCare Connects seeks a skilled and dedicated Occupational Therapist to deliver exceptional care and support adults of all ages in the comfort of their own homes.

This independent contractor role serves the Ladera Ranch area and surrounding communities.

Key Responsibilities:
• Conduct thorough assessments and evaluations to identify patient care needs, focusing on functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
• Develop personalized treatment plans, implement interventions to enhance patient wellbeing, and continuously evaluate progress.
• Provide services in accordance with state laws governing Occupational Therapy, adhering to the plan of care and collaborating with other healthcare team members.
• Lead patients in engaging therapeutic, creative, and self-care activities to improve functionality and safety.
• Monitor, record, and report patient responses to treatment and changes in patient condition.
• Educate patients, families, caregivers, and healthcare team members on therapy areas where they can participate in assisting patients.

Requirements include:
• Ability to work effectively with individuals of all ages
• Active Occupational Therapist license and registration by the state
• Completion of an accredited Occupational Therapist program
• Bilingualism is a valuable asset
