Job Overview
Mass General Brigham is seeking a skilled and dedicated Clinical Nurse Leader to join our team as a Home Health Charge Nurse (RN). Located in Malden, Massachusetts, this role will provide top-notch nursing care to patients in various settings within the community. As a member of our interdisciplinary team, you will work collaboratively with other healthcare professionals to deliver high-quality patient care.

About the Role
This 36-hour per week position offers a competitive salary of approximately $120,000 per year, commensurate with experience. The successful candidate will have a current Massachusetts license as a Registered Nurse and at least one year of recent nursing experience in an acute or emergency setting. Home care experience is a plus, as is the ability to travel within designated geographic areas. Strong interpersonal and customer service skills are essential, along with the ability to communicate effectively in writing, verbally, and electronically.

Responsibilities
- Develop and implement individualized plans of care that prioritize patient needs, safety, and diagnosis for optimal outcomes.
- Provide education to patients and families, adapting teaching methods to meet their unique learning styles and needs.
- Collaborate with multidisciplinary teams to ensure seamless transitions and referrals to other disciplines and community resources.
- Maintain accurate and timely electronic documentation, reconciling medications and updating patient information as needed.
- Rotate charge nurse responsibilities, ensuring continuity of care and effective communication among team members.
