This position allows you to leverage your expertise and compassion to make a direct difference in the health and well-being of others.

You will have the opportunity to work in a variety of venues, including home visits, skilled facilities, and telehealth visits, providing services that cater to the unique needs of each individual.

Your Responsibilities

  • ">
  • Conduct adult/geriatric assessments, medical history reviews, diagnoses, and treatment plans.">
  • Work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals as needed.">
  • Provide patient education and follow-up support, working closely with Primary Care Physicians (PCPs) to ensure seamless care.">
  • Offer services in various settings, including home visits, skilled facility visits, and telehealth visits.">

What We Offer

A competitive salary of $95.00 per completed assessment, with opportunities for flexible scheduling and mileage reimbursement.

As a PRN/part-time provider, you will be non-benefited except for 401(k) and limited voluntary benefits, with a focus on autonomy and flexibility in your work schedule.

Qualifications and Requirements

To be considered for this role, you must possess:

  • ">
  • A Master's degree or equivalent experience and licensure as an NP or PA.">
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain it.">
  • Board certification in a Matrix-approved specialty.">
  • 1 year of experience as a Nurse Practitioner/Physician Assistant, with strong computer skills and familiarity with EMR software.">
  • Excellent verbal and written communication skills, with a positive and outgoing personality.">
  • A valid state driver's license and ability to travel, with a focus on meeting patients in their homes.">
