As a Field Community Health Worker, you'll play a vital role in ensuring that Medicaid members receive the care they need. Your liaison role will involve providing home and social screenings, care coordination, and member education to support their health and well-being.
As a Field Based Community Health Worker, you'll travel 50% of the time in Asheville, NC and surrounding areas within 75 miles. Your schedule will be Monday-Friday, standard business hours, with no nights, weekends, or holidays. You'll have the flexibility to work 4 10-hour days (7 am-6 pm) if you meet certain metrics and demonstrate competency in the role.
Key responsibilities include:
- Linking members to applicable community resources
- Completing field visits for high-risk, hard-to-reach members
- Telephonic outreach to targeted members
- Supporting regular visits to their primary physician to keep them out of the hospital
- Providing member education and supporting transitions of care
- Collaborating with the member's care team to ensure a positive and engaging experience
Required qualifications include a High School Diploma or GED, 1+ year of experience with knowledge of community culture and values, and familiarity with local resources. You'll also need a designated workspace with high-speed internet, reliable transportation, and a clean NC driver's license. Preferred qualifications include a Bachelor's Degree in Social Work or Healthcare Administration, or experience working in Managed Care.
