Care Coordinator in High Country Healthcare

About the RoleWe're seeking an experienced Social Worker with a passion for patient care to join our team as a Care Coordinator. In this role, you'll serve as a coordinator of interdisciplinary approaches to guide patients through the continuum of care, focusing on discharge planning, psycho-social needs assessment, care/resource management, and provider/insurance issues.
You'll collaborate with physicians, patients, their family members, and others to facilitate desired outcomes, address ongoing needs, maximize patient, family, and provider satisfaction, optimize systems/resource utilization, facilitate timely discharge, and reduce unnecessary delays.
Your decisions and actions will be guided by compassion and respect for each individual's inherent dignity, worth, and unique attributes. You'll effectively communicate with patients/families, visitors, colleagues, physicians, leadership, and volunteers.
Key Responsibilities- Serve as a coordinator of interdisciplinary approaches to guide patients through the continuum of care.
- Collaborate with physicians, patients, their family members, and others to achieve desired outcomes.
- Facilitate communication among healthcare professionals and patients/families.
- Assist with discharge planning and ensure timely discharges.
You'll work closely with our multidisciplinary team to provide high-quality patient care and promote positive outcomes.
Requirements- Bachelor's degree in Social Work from an accredited college or university required; Master's degree in Social Work preferred.
- Previous experience in Care Management is preferred.
- Certification in Care Management is preferred but not required.
- Demonstrated knowledge of and works within the scope of practice of a social worker.
- Excellent communication and interpersonal skills.

Salary and BenefitsThe salary for this role is estimated to be around $50,000 - $60,000 per year, based on location and experience. Additionally, you'll be eligible for our comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
