Job DescriptionThis role involves delivering high-quality primary care services as part of an integrated care team. Key responsibilities include determining health histories, performing physical exams, ordering and interpreting diagnostic tests, and developing person-centered care plans. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for providing compassionate patient care.
- Determine and formulate health histories and perform physical exams.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems.
- Collaborate with patients and the care team to develop and implement effective care plans.
To be successful in this role, you will need:
- A graduate degree from an accredited Nurse Practitioner Program.
- A minimum of 3 years of experience working as a Nurse Practitioner, preferably with a background in mental health and addiction medicine.
- Excellent clinical skills, including the ability to diagnose and treat minor and acute illnesses.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
