Job Title: Hospice Clinical Manager

VNA Health is seeking a skilled Hospice Clinical Manager to join our team. As a key member of our hospice care team, you will be responsible for ensuring that patient care is coordinated and managed appropriately.

Key Responsibilities:
  • Monitor and evaluate patient care through review of nursing and secondary staff documentation, medications, patient care plans, and interventions.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing plans of care.
  • Participate in patient admission cases and complete accurate, timely employee performance evaluations.
  • Develop and implement a preceptor and/or mentoring environment for hospice staff.
  • Enhance and maintain nursing orientation for RN case managers, staff nurses, and secondary discipline staff.
Requirements:
  • Current and active California RN license.
  • Bachelor's degree, preferred.
  • Two years management or supervisory experience in a healthcare setting, preferably hospice/home care.
  • Current CPR certification.
  • Licensed driver with insured automobile.
