Job Opportunity

We are currently seeking a highly skilled Physical Therapy Professional to join our team as a per diem member for our Kids on the Move program in Bennington County.

Job Summary

As a Physical Therapy Professional, you will be responsible for evaluating, planning, and administering medically prescribed or educationally relevant physical therapy treatment to clients in their homes or students in their respective educational environment.

You will work under the direction of the Kids on the Move Clinical Manager and be responsible for clinical supervision of students, physical therapy assistants, and rehabilitation aides.

Main Responsibilities
  • Maximizing Productivity

Actively monitor and amend patient visit schedules to maximize productivity and patient need. This includes taking action to account for new patients and changes in existing patient schedules, including calling patients to reschedule visits, reshuffling patient loads to meet new demands, communicating recommended changes to supervisors, etc.

Clinical Assessments

Perform comprehensive PT assessments of assigned patients, including psychosocial, educational, safety, and risk factor parameters, as evidenced by clinical documentation and in accordance with federal and state regulations, organizational and departmental policies, and standards of care, and APTA guidelines.

Developing Plans of Care

Develop plans of care to promote optimal functional independence and safety, as documented by clinical records in accordance with federal and state regulations, organizational and departmental policies, and standards of care, and APTA guidelines.

Providing Skilled Treatment

Plan for and provide skilled physical therapy treatment, including therapeutic techniques, patient/caregiver education, assessing patient status and progress on each visit, and ensuring quality of care, as documented in clinical records and in accordance with federal and state regulations, organizational and departmental policies, and standards of care, and APTA guidelines.

Coordinating Patient Care

Coordinate patient care with other clinical team members, including physicians, nurses, caregivers, other rehab services, and students, ensuring optimal patient care and communication, as noted in clinical records.

School-Based Services

Accurately and completely perform functions related to school-based services, including evaluations, documentation, development of Individual Educational Plans (IEPs), participation in meetings, and provision of direct and consultative services. Fill out and complete insurance paperwork in a timely manner and in accordance with applicable regulations.

Consultations and Follow-Ups

Actively provide services in accordance with Vermont State Department of Health, including clinic consultations, follow-ups, seating and mobility assessments, etc.

Supervising Clinical Activities

Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations, department and organizational policies, and standards of care, and APTA guidelines.

Completing Required Paperwork

Complete and coordinate required paperwork in accordance with federal and state regulations, organizational and departmental policies, standards of care, and APTA guidelines to ensure that documentation reflects care provided and outcomes achieved, as evidenced in clinical records.

Billing and Charging Processes

Complete required paperwork to ensure charging and billing processes are accurately executed, including documents that support charges entered, care within insurance coverage parameters, required insurance forms completed, etc.

In-Service Training and Marketing

Participate in in-service training programs for customers, including Rehabilitation, Medical, Nursing staff, volunteers, community groups, etc. Proactively engage in the marketing and promotion of Rehab services, including developing specialized areas of service, participating in public health fairs, medical community, etc.

Quality Improvement

Work collaboratively with QI staff to maximize accuracy of clinical notes and OASIS documents.

Mckesson Processes

Follow agency protocol with McKesson processes, including locking-down visits, follow-up notes, visit completion, etc.

Patient Discharges and Transfers

Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork and patient information is processed.

Home Care Services

Serve as a Home Care PT as required.

Qualifications and Experience
  • Degree and Licensure

Degree in physical therapy required. Current Vermont State PT license required.

Valid Driver's License

Valid driver's license.

Experience

Two years of clinical physical therapy experience preferred.

Compensation and Benefits

The estimated annual salary for this position is $85,000-$100,000, depending on qualifications and experience.
