Salary and Benefits

The estimated salary for this role is $37.40-$39.60 per point, depending on the day of the week and the type of visit. In addition to a competitive salary, you will also receive:

Job Description

As a Home Health LPN, you will be responsible for educating patients and their families on proper care techniques, medication administration, and health promotion strategies. You will communicate effectively with patients, families, and healthcare providers to ensure continuity of care and positive outcomes. Your duties will include:

  • Educating patients and their families on proper care techniques, medication administration, and health promotion strategies.

  • Participating in ongoing training and professional development opportunities to enhance your skills and knowledge.
  • Providing one-on-one skilled nursing care and treatment to patients in their home.
  • Assisting the registered nurse in assessment of patient's condition and contributing information to aid in planning and evaluating care.
  • Demonstrating knowledge of home health and competencies in discipline-specific patient care skills required by the care center to provide patient care.
  • Performing comprehensive patient assessments and collaborating with a multidisciplinary care team to develop an individualized plan of care as per physician orders that achieve desired outcomes and goals.
  • Checking patient's vital signs, performing wound care, and educating patients and their families about their medications and chronic conditions.
  • Providing patient episode management per protocol.
  • Completing documentation timely and accurately according to industry standards, conventions, and guidelines, including subsequent visits, or other required documentation.
Requirements

To be considered for this role, you must have:

  • A current and unencumbered license as a Licensed Practical Nurse (LPN)
  • Strong assessment, critical thinking, and communication skills.
  • 1+ year(s) of Certified Home Health experience as an LPN
  • Ability to work independently and as part of a dynamic team.
  • Comfortability using technical tools and software/EMR for visit documentation.
  • Current CPR certification.
  • Reliable transportation, current driver's license, and car insurance coverage.
  • Proof of Immunization/Exemption for Flu and COVID-19.
  • TB Test or Chest X-Ray.
