Assisting Hands Home Care is seeking a compassionate and dedicated Field Nurse to provide high-quality care to our clients in Hudson and parts of Bergen counties. As a Field Nurse, you will be responsible for conducting thorough client assessments, developing comprehensive care plans, and ensuring the delivery of proper care adhering to RN standards.
Responsibilities:
- Conduct thorough evaluations of client needs.
- Develop comprehensive care plans tailored to each client.
- Perform 30-day check-in evaluations and 60-day re-assessments.
- Oversee and ensure the quality of care provided.
- Provide essential education to clients and caregivers.
- Maintain regular communication with patients, families, caregivers, pharmacies, doctors' offices, and medical services.
Qualifications:
- Current New Jersey Registered Nurse (RN) license, preferably with a BSN.
- Proven experience in patient care, with a preference for Home Care.
- Valid driver's license and reliable mode of transportation.
- Able to travel to clients' homes throughout Hudson and parts of Bergen counties.

Compensation: $51.00 per hour
