Care Coordinator and Floor Nurse for Seniors

About the RoleThis is a full-time position that combines nursing responsibilities with MDS coordination. You will work closely with the Director of Nursing and other disciplines to complete Minimum Data Set (MDS) assessments and manage resident care planning processes. Your strong communication skills and knowledge of Medicare, case-mix processes, and regulatory compliance will be essential in optimizing reimbursement for services provided.

Key Responsibilities- Collaborate with the Director of Nursing and other healthcare professionals to complete MDS assessments and manage resident care planning processes.
- Ensure compliance with federal regulations and optimize reimbursement for services provided.
- Manage resident assessments and tracking of all Medicare/Managed Care/Medicaid case mix documents.
- Assist with the coordination of resident care planning processes under the direction of the Director of Nursing or CRM (where applicable).
- Graduation from a School for Registered Nursing required.
- Minimum 1 year of previous experience in an MDS/Clinical Reimbursement Nurse role preferred.
- Current State Licensure as a Registered Nurse in the state where practicing is required.
- Current CPR and Automated External Defibrillator (AED) certifications required.
