Eyesight Expert

As an Independent Optometrist at Sam's Club, you will lead patient care for eye and vision needs, as well as general health and well-being.

You will:

• 
  
• Establish your own days and hours of practice
  
• Control your patient schedule and set your fee schedule
  
• Keep 100% of exam/professional fees
  
• Have the option to participate in equipment/instrumentation and administrative/Opt-Tech support services (if state allows) for a fair market value
  

To be eligible for this opportunity, you must:

• 
  
• Hold a Doctor of Optometry (OD) degree from an accredited optometry school
  
• Be a licensed Optometrist (OD) in good standing
  
• Obtain a General Liability Certificate and Professional Liability Insurance Certificate
  
• Provide a valid W-9
