Healthcare Professional for Elderly Care

Seeking a dedicated Nurse Practitioner to provide exceptional patient care in a fast-paced outpatient setting. As a member of our interdisciplinary team, you will be responsible for delivering medical services in a PACE facility. With a strong focus on community clinic experience and managed care systems, we value professionals with a passion for working with the elderly.
About the RoleThis opportunity involves assessing and diagnosing a variety of diseases and injuries in patients of the elderly. You will perform therapeutic procedures, including preventative care and follow-ups, and arrange referrals, consultations, and therapeutic services as needed. Additionally, you will chart review and document all patient encounters efficiently and according to organization policy and JCAHO standards.
We are seeking an experienced professional who can coordinate the preparation of reports and written documentation, including medical records, as required by the department clinic, state, and federal agencies. This role also involves supervising nurses and assistants in the examining room and may serve as a preceptor for team members.
Requirements- CA medical license as NP
- CA DEA certificate before start date
- Board certified
- CPR certification
- Demonstrated experience in community clinic and/or Managed Care System
- Experience working with the elderly
- 2 years of experience
A Spanish fluency is preferred but not required.

CompensationThe estimated salary for this position is $104,000 - $156,000 annually, depending on experience. Benefits include:
