About the Role

At Virtua Health, Inc., we are seeking a dedicated and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. This is an exciting opportunity to work in a Magnet-recognized health system with five hospitals, seven emergency departments, and more than 280 locations in South Jersey.

We offer a competitive salary of $75,000 - $90,000 per year, depending on experience.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to ensure high-quality outcomes.
  • Coordinate patient care across various disciplines, including nursing, therapy, and social services.
  • Educate patients and their families on disease management, self-care techniques, and wellness strategies.
  • Collaborate with healthcare teams to identify and address gaps in care, promoting seamless transitions and optimal patient experiences.
  • Communicate effectively with patients, families, and interdisciplinary teams to ensure comprehensive care coordination.
Requirements
  • Graduate of an accredited school of nursing with a valid NJ RN License.
  • Demonstrated expertise in nursing skills, hospital practices, and current standards of care.
  • Ability to prioritize tasks, manage time efficiently, and maintain accurate records.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
