About the Role:
The successful candidate will be responsible for conducting home health aide skills trainings, competency evaluations, and creating personalized care plans for our clients. This role requires strong communication and organizational skills, with the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct Home Health Aide skills trainings, competency evaluations, HHA introduction, and 90-day evaluations
  • Create initial client assessment, plan of care, 90-day reassessments, and ongoing evaluations of the GAFC setting
  • Participate in care plan creations and reviews, along with discharge plans
  • Assess and address motivational, psychosocial, and environmental concerns
  • Maintain and update client charts, medical records, and submit all relevant information to authorized organizations

Requirements:
We are looking for an experienced LPN with a valid Massachusetts license and a minimum of 1 year experience in an acute or home health care setting. Fluency in English and a second language is preferred, as well as experience in case management, social services, and community health work. The ideal candidate will have strong communication, writing, organizational, and time management skills, with the ability to multitask and work in a fast-paced environment.

Benefits:
Vitra Health offers a competitive salary range of $65,000-$80,000 per annum, depending on experience, as well as a range of benefits including health insurance, dental and vision benefits, supplemental benefits, 401K, paid-time-off, and career growth opportunities.
