We are currently seeking a skilled Licensed Vocational Nurse to join our team.

As a key member of our interdisciplinary team, the LVN/LPN will be responsible for providing high-quality patient care in the comfort of clients' homes.

Responsibilities:

  • Maintain accurate and up-to-date patient records to ensure seamless communication among caregivers.
  • Recognize and respond to symptoms, implementing necessary interventions within the scope of an LPN/LVN in California.
  • Administer medication and treatments as prescribed by physicians.
  • Foster patients' comfort and well-being by addressing personal hygiene needs, nutrition, hydration, and supply management.
  • Provide education, support, and guidance to patients, families, and caregivers.
  • Collaborate with the interdisciplinary team to ensure comprehensive patient care.

Qualifications:

  • Degree from an accredited practical nurse or vocational nursing program.
  • At least one year of nursing experience, preferably in community health, hospice, medical/surgical, or homecare settings.
  • Current LPN/LVN licensure in California.
  • Excellent communication, organization, and problem-solving skills.
