We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Abode Care Partners. About the Role

This House Calls Nurse Practitioner position offers an exceptional opportunity to grow your medical skills while delivering personalized care to seniors and patients with complex conditions. You will work collaboratively with caregivers and healthcare professionals to maintain a comprehensive plan of care, ensuring the highest level of patient satisfaction and well-being.

Key Responsibilities
  • Plan, direct, and evaluate patient medical needs, ensuring evidence-based care and timely interventions.
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Order and interpret laboratory and diagnostic testing as needed, collaborating with specialists when necessary.
  • Maintain accurate and up-to-date patient medical records, adhering to HIPAA regulations.
Requirements
  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
