Job Title: RN Care Coordinator Achievable Dream
We are seeking a highly skilled and compassionate Registered Nurse to join our team as a RN Care Coordinator Achievable Dream. In this role, you will have the opportunity to make a positive impact on the lives of students and staff members while maintaining a healthy work-life balance.
About the Role
As a RN Care Coordinator Achievable Dream, you will partner with the Health Services Supervisor to provide nursing care for students and staff, ensuring a supportive and balanced work atmosphere. Your responsibilities will include collaborative care, health screenings and assessments, community wellness initiatives, wellness programs, education and counseling, and medication administration and clinic management.
Key Responsibilities
• Collaborative Care: Partner with the Health Services Supervisor to provide nursing care for students and staff, ensuring a supportive and balanced work atmosphere.
• Health Screenings & Assessments: Perform and interpret health screenings, assess healthcare needs, and develop care plans that align with student and staff goals.
• Community Wellness Initiatives: Participate in health fairs, educational programs, and events that engage and uplift the local community.
• Wellness Programs: Initiate and implement health programs based on clinic needs, empowering individuals to reach their wellness objectives while allowing you the flexibility to focus on your own.
• Education & Counseling: Deliver classroom education, provide health counseling, and create a positive, stress-free learning environment for students and staff.
• Medication Administration & Clinic Management: Administer medication and manage clinic supplies, all while prioritizing efficiency and maintaining balance in your daily tasks.
Qualifications
This role requires a nursing program graduate with a Bachelor's degree in Nursing, 1 year of experience in well care or community health, and Registered Nurse (RN) licensure from the Virginia Department of Health Professions (VDHP). Additionally, you must hold CPR/BLS certification from the American Heart Association/American Red Cross (AHA/ARC).

Compensation and Benefits
This role offers a competitive salary of $65,000 - $75,000 per year, depending on experience. Additionally, you will be eligible for a sign-on bonus of up to $5,000. Our comprehensive benefits package includes medical, dental, and vision insurance, 401(k) retirement plan, and paid time off.
