Overview

Salary: $85,000 - $110,000 per year

About the Role

We are looking for a highly motivated and organized individual to coordinate patient care and facilitate communication among healthcare providers. The successful candidate will possess strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality patient care.

Key Responsibilities

- Coordinate patient care plans with interdisciplinary teams
- Facilitate effective communication among healthcare providers
- Ensure timely and accurate documentation of patient information
- Collaborate with nursing staff to implement evidence-based practices
- Develop and maintain relationships with patients and families to promote positive outcomes

Requirements

- Earned Master's degree in Nursing or related field
- Current licensure as an Advanced Practice Registered Nurse (APRN) in Tennessee
- 2+ years of experience in a healthcare setting, preferably in a leadership or coordination role
- Strong clinical skills and knowledge of adult medical-surgical, pediatric, and critical care principles
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
