LPN Job Description

We are seeking a skilled LPN to join our team at Providence Medford Medical Center. As a LPN, you will play a vital role in delivering high-quality patient care and making a positive impact on our patients' lives.

Key Responsibilities:
  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans to meet patients' unique needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care.
Requirements:
  • Graduate of an approved school for practical nurses.
  • Oregon Licensed Practical Nurse.
  • National Provider BLS - American Heart Association.
  • 1 year LPN Experience.
