Job Overview

We are seeking a highly skilled Practical Nurse Leader to join our team at Yale New Haven Health. As a key member of our patient care team, you will play a critical role in delivering exceptional care to our patients.

About the Role

This is an exciting opportunity for a licensed practical nurse (LPN) to lead and supervise nursing care on our unit. The successful candidate will have a strong background in rehabilitative, psychiatric, or geriatric nursing and be licensed in the State of Connecticut.

Responsibilities
  • Leadership and Supervision: Direct and supervise the nursing care and related activities on the unit, ensuring compliance with facility policies and procedures.
  • Communication and Collaboration: Maintain open communication with nursing staff, physicians, families, and other stakeholders to ensure seamless care coordination.
  • Clinical Expertise: Provide high-quality patient care, including medication administration, wound care, and data collection, while working closely with registered nurses to develop comprehensive care plans.
  • Documentation and Reporting: Accurately document patient information, medications administered, and treatments provided, as well as participate in resident care conferences and contribute to employee evaluations.
  • Safety and Infection Control: Identify and address safety hazards, implement infection control measures, and maintain a clean and safe environment for patients and staff.
  • Education and Training: Welcome new staff members, provide preceptorship, and engage in continuing education opportunities to enhance knowledge and skills.
Requirements
  • Education: Graduation from an approved Practical Nurses Education Program.
  • Experience: At least one year of work experience in rehabilitative, psychiatric, or geriatric nursing preferred.
  • Licensure: A Licensed Practical Nurse licensed in the State of Connecticut.
  • Skills: Excellent communication and interpersonal skills, ability to maintain standards of care, and adaptability in a fast-paced environment.
Compensation and Benefits

The estimated salary for this role is $65,000 - $80,000 per year, depending on experience. We offer a comprehensive benefits package, including health insurance, retirement plans, and paid time off.
