About the Role

We are seeking a highly skilled Clinical Social Worker to join our team as a Care Coordinator in our Social Work Clinic. This is a full-time, 40-hour position that will work closely with high-risk patients to establish, implement, monitor, and evaluate high-quality, cost-effective care plans.

The ideal candidate will have a strong understanding of psychiatric and family systems, as well as the ability to work with patients of all ages who suffer complex medical and psychiatric problems. They will also be able to communicate effectively with patients, families, and healthcare clinicians throughout the continuum.

The Clinical Social Worker will be responsible for:

  • Completing comprehensive biopsychosocial assessments of patients to evaluate clinical needs
  • Developing and implementing comprehensive care plans
  • Providing direct and ongoing care management to select patients
  • Collaborating with the care team to ensure timely implementation of care plans

The successful candidate will have a Master's degree in Social Work (MSW) and be licensed as a Licensed Independent Clinical Social Worker (LICSW) or be working towards LICSW licensure. They will also have at least five years of experience in the field of psychiatry, substance abuse, trauma, and/or community mental health services.
