Job Summary
We are seeking a skilled Care Coordinator Specialist to join our team at Volunteers of America-Greater New York. In this role, you will work with homeless individuals with substance use disorders to provide them with access to treatment, benefits, and housing.
Key Responsibilities
- Conduct outreach and engagement visits to Wellness Centers, Safe Havens, Shelters, and other areas within the catchment area.
- Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
- Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings.
- Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals.
- Provide supportive counseling to strengthen clients' ability to achieve behavior change and other goals.
- Promote a successful transition to permanent housing through skills-building, case management, and counseling.
Requirements
- A minimum of 60 college credits (BA preferred) plus two years of relevant work experience.
- Familiarity with homelessness, mental health, and/or substance abuse population required.
- Demonstrated ability in verbal and written communication, including basic computer skills.
