Job Summary

We are seeking a highly skilled and compassionate Social Services Aide/Designee to join our team at Alden Estates of Orland Park. As a key member of our Social Services Department, you will play a vital role in ensuring the well-being and quality of life for our residents.

Responsibilities
  • Assist the Social Service Director in planning, developing, and organizing the Social Services Department to meet individual resident needs and facility goals.
  • Conduct initial and ongoing assessments, including social, medical, cognitive, physical, and behavioral evaluations, to identify resident needs and develop personalized care plans.
  • Collaborate with the Interdisciplinary Team to develop and implement comprehensive care plans that address resident needs and promote overall well-being.
  • Provide supportive counseling and behavioral interventions to residents, as needed, and document progress and outcomes.
  • Act as a resident advocate, liaison between residents and families, and facilitate communication between the facility and community agencies.
  • Develop and maintain relationships with local senior services, organizations, and healthcare providers to promote the Social Services Program and enhance resident care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure compliance with state and federal regulations.
Requirements
  • Preferred Bachelor's Degree in Human Services or related field; High School Diploma required.
  • Two years of experience in social services or case management in the healthcare field.
  • Current knowledge of Federal and State Long-Term Care Social Service regulations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and staff.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Leadership skills and ability to work collaboratively as part of a team.
