Job Summary

The primary role of the Medical Social Worker II at Kaiser Permanente is to assist patients and their caregivers in coping with social, emotional, and practical issues related to their illness. As a key member of the home health team, you will deliver age-appropriate social work care to patients in their place of residence, ensuring that they receive comprehensive support.

Responsibilities:
  • Provide psychodynamic interventions, crisis intervention, grief counseling, problem-solving, stress reduction, and healthy coping strategies to individuals, families, and groups.
  • Counsel patients on managing acute, chronic, or terminal illnesses and develop individualized plans to address their needs.
  • Conduct biopsychosocial strengths-based assessments to identify areas for improvement and implement effective treatment plans.
  • Collaborate with patients to identify needs and develop mutually agreed-upon treatment plans.
  • Discuss care options proactively, including Kaiser resources and external community/government services, to help patients and families develop short- and long-term care plans.
  • Work with other disciplines to assess, plan, and provide services for patients using biopsychosocial information.
  • Assist patients in advocating for themselves to receive appropriate services within Kaiser and the community.
Requirements:
  • A strong understanding of psychosocial theories and principles.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively with diverse populations, including children, adults, and families.
  • Familiarity with state and federal regulations governing home health and hospice care.
  • Strong organizational and time management skills.
