Patient Care Manager with Excellent Communication Skills

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at North Side Christian Health Center.
Job SummaryThis is an exciting opportunity for a dedicated healthcare professional to provide exceptional patient care, support clinical teams, and contribute to the overall success of our organization.
Key Responsibilities:- Clinical Support: Assist physicians with patient assessment, evaluation, and education, ensuring timely and effective care delivery.
- Case Management: Coordinate patient care with internal and external resources, promoting seamless transitions and optimal outcomes.
- Patient Education: Develop and deliver comprehensive patient education programs, empowering individuals to manage their health effectively.
- Communication: Foster open dialogue with patients, families, and healthcare teams, ensuring clear communication and empathy in all interactions.
- Team Collaboration: Work collaboratively with multidisciplinary teams to achieve shared goals, improve patient satisfaction, and enhance the quality of care.
- Education: Current RN licensure in Pennsylvania, CPR certification (child and adult), and BLS required.
- Experience: Previous experience in outpatient and/or community settings preferred, with expertise in pediatrics and adults highly desirable.
- Skill Set: Strong computer skills, including Microsoft Office Suite, excellent communication and interpersonal skills, and ability to work in a fast-paced environment.
- Competitive Salary: $65,000 - $80,000 per year, based on experience.
