Job Summary

We are seeking a highly skilled Patient Care Coordinator - Interpreter to join our team at Heart of Ohio Family Health Centers.

About the Role

The successful candidate will provide exceptional patient care and support, ensuring seamless communication between patients and healthcare providers. This role requires strong interpersonal and organizational skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Patient Liaison: Function as a liaison between patients and healthcare providers, facilitating effective communication and coordination of care.
  • Interpretation Services: Provide accurate interpretation services in English and other languages, enabling patients to fully participate in their care.
  • Referral Coordination: Research, document, and inform patients about available health resources, ensuring timely referrals and follow-up appointments.
Requirements:
  • Education: Associate degree or higher, preferably in a related field. Experience with healthcare referrals is an asset.
  • Certifications: Hold a interpreting certificate or equivalent experience.
  • Cultural Competence: Demonstrate the ability to work with diverse cultural and socioeconomic groups, without judgment or bias.
  • Communication Skills: Excellent verbal and written communication skills, with the ability to convey complex information clearly and concisely.
Work Environment:

This role is based in our medical office environment, with a focus on providing exceptional patient care and support. The ideal candidate will be comfortable working in a dynamic setting, with a strong emphasis on teamwork and collaboration.

Estimated Salary: $45,000 - $55,000 per year, depending on experience and qualifications. We offer a competitive benefits package, including medical, dental, and vision coverage, as well as opportunities for professional growth and development.
