We are seeking a dedicated Senior Social Worker to join our interdisciplinary team at a skilled nursing facility. This is an exceptional chance to make a meaningful impact in the lives of seniors while enjoying a fulfilling career with opportunities for growth.

Key Responsibilities:

  • Provide initial assessments and develop personalized care plans to address mental health issues and ongoing support needs
  • Collaborate with healthcare professionals to ensure seamless care coordination and effective communication with residents and their families
  • Conduct regular evaluations to monitor progress, adapt treatment plans as needed, and maintain accurate documentation
  • Foster open communication with the facility team and Regional Clinical Manager to drive clinical program success
  • Utilize expertise in Mental Health diagnoses and therapeutic approaches to promote resident well-being

Requirements:

  • Master's degree in Social Work and valid LCSW license in the state of employment
  • 2+ years of counseling and therapy experience, preferably in a healthcare setting
  • Strong medical knowledge, understanding of documentation (CMS, Medicare, Medicaid), and experience working in skilled nursing facilities
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment

Benefits:

  • $85,000 - $95,000 per year, based on experience and location
  • 5 weeks of paid time off, including personal, sick, CME & holidays
